Editorial Note

In this issue of the digest, Anil Varshney requests readers to provide the prices of vaccines purchased by their governments for a WHO-SEARO study. Umit Kartoglu shares the final draft of Model requirements for the storage and transport of time and temperature sensitive pharmaceutical products for general public review. Further, there is more on the vaccine freezing discussion and a job announcement.

WHO-SEARO is conducting a study entitled "Vaccine price trends in the past decade and future projections" for vaccines purchased/sold in developing countries with special reference to the SEARO countries.

The list of vaccines includes:
Rubella, Measles, MMR, MR, Hep B monovalent, Hep B tetravalent, Hib monovalent, DTP, Hib pentavalent, HPV, Pneumococcal, Rotavirus, and SA14-14 2 JE vaccine live attenuated.

This is to request TechNet21 members to provide the prices of the vaccines that are purchased by their national governments. It would also suffice if members could provide the sources from which the prices of the above vaccines could be obtained.

The World Health Organization (WHO) today released its final draft of Model requirements for the storage and transport of time and temperature sensitive pharmaceutical products for general public review. Review and comment period is open until July 31st, 2010.

This long-awaited guideline, prepared in close consultation with the WHO Task Force on Regulatory Oversight on Pharmaceutical Cold Chain Management, outlines the principal requirements for the safe storage and distribution of time and temperature-sensitive pharmaceutical products. It is based upon existing regulations and best practice guidance from a wide range of international sources, while accepting that local legislation and regulations will continue to take precedence. The final version will be prepared incorporating comments received from this public review. The document will then be submitted to Experts Committee on Biological Standardization and Experts Committee on Specifications for Pharmaceutical Products for their approval in October 2010.

... Studies have shown that live oral polio vaccine can be subjected to repeated freeze-thaw cycles without a loss of potency (for an example, see WHO Weekly epidemiological record, 1990, 65: 207-210). However, a critical point in this study was that the 'high' temperature was still within the range of a working refrigerator (in the case of the study cited, this was +2.1 C).

Exactly why live OPV is resistant to damage by freezing is less clear. Current formulations of OPV contain stabilizers, usually a high concentration of magnesium chloride.  Divalent cations such as Mg++ bind to proteins and stabilize their structure. This could presumably explain the protection against freezing. Magnesium chloride also lowers the freezing point of the vaccine to -6 to -8 C.
